Initial intake visit (which can be virtual as long as you live in a state Modern Endocrine has a license in) with a health coach or provider to oversee your intake information and order any labs you need to have drawn. This is done to minimize your waiting time in between your first intake appointment and your follow up lab review appointment. YOU CAN USE YOUR INSURANCE FOR LABS IF YOU WOULD LIKE OR WE OFFER DISCOUNTED CASH PAY RATES. We prefer all labwork be updated by us as we order in depth testing to ensure your weight loss journey is optimized. We focus on cortisol and insulin imbalance along with lowering inflammation systemically and in your gut as some of the root causes of weight gain. If your visit is in person an InBody Exam will be preformed with macros and nutrition suggestions set up.
GI MAP and Food Sensitivity Testing (included in your cost for the program which is $1268 value) and will be sent directly to your address on file. A health coach will review the results of these tests with you after your plan is made by a provider
Lab review follow up with provider to discuss results and formulate the best comprehensive weight loss plan for you
Monthly visits (in person or virtual) with health coach to ensure accountability and adjust weight loss plan as needed
Unlimited access to health coach via online HIPPA compliant portal
Access to patient education materials about weight loss including a copy of Dr. Smith’s ebooks 10 Stages to Ultimate Health, Cheat Sheet Superfoods, & Better Sleep and Better Health
Provider follow up every 2 months to continue to redefine weight loss plan and long term goals
Labs will be repeated at the end of your program (AGAIN YOU CAN USE INSURANCE OR WE OFFER DISCOUNTED CASH PAY RATES) and you will review these with your provider at your last visit
